I am confused by a 2011 Form 5498 that I have received from Pinnacle Bank. The form is clearly marked as “2011.” The only entry is in Block 5. The notes on the form state that Block 5 “Shows the FMV of all investments in your account at year end.”

The amount shown corresponds to the value of my TIRA as of the end of 2010. It seems to me that this value as of Dec 31, 2010 should be shown on a 2010 Form 5498 rather than on a 2011 Form 5498. Am I incorrect?
