Form 8606 for Roth Indirect Rollover from 401(k)

Client is over 65 and took an NUA Distribution.  Remaining 401 (K) Qualified assets were rolled to an IRA.  Client elected to move after tax funds to a Roth IRA but forced by record keeper to take the funds himself first and therefore did an indirect rollover to an established Roth IRA within 60 days.

Client did no other indirect rollovers in any IRA in the same calendar year.

Question – must the client file a form 8606 to address the 60 day rollover?  My assumption is not since the source of the funds is a 401(k).

Any help or guidance would be greatly appreciated.



Form 8606 does not apply. The rollover to a Roth IRA is only reported directly on lines 16a and 16b of Form 1040 with “Rollover” entered next to 16b.
