does large ROTH conversion cause limitation to ROTH contribution
My wife and I made 140k ROTH conversion in 2018. Combined with our W-2 and pension incomes, that puts us in the AGI above 260k. unfortunately, we also made full ROTH contributions of 13k total. Did that cause problems with IRS? if it does, how can we fix it?

Not a problem. Roth conversions are not counted in your AGI for purposes of the income limit for making regular Roth contributions.
