Whereas a QCD is not included in AGI, is a QCD counted in the “Medicare MAGI” calculation? i.e. the Modified AGI that Medicare uses to determine IRMAA premium adjustment?

No, it is not included. It reduces MAGI for IRMAA purposes compared to a distribution of the same amount that is not a QCD.
