If there was no contingent beneficiary, then the plan document must be consulted to determine what the default provisions provide. With no beneficiary, the most common default beneficiaries are either the estate or any surviving spouse.
If it is the estate, the will would determine the estate beneficiary and if there is no will, the state intestate provisions will determine the beneficiary.
