60 day rollover and uncashed check
If the 401(k) company has issued a distirbution check more than 60 days ago but the check has never been cashed, can it still be rolled into an IRA? Does the 60 days start on the check date, the receipt date, or the cash date?

If the check is made out to the employee, the 60 days starts with the receipt date of the check. If the check is made out as a direct rollover check to the IRA custodian FBO the employee, then there is no 60 day limit.
