beneficiary ROTH IRA

It is my understanding that when a beneficiary goes to re title the ROTH IRA they must include the name and date of death in the new ROTH IRA title for them to get the stretch payout from the ROTH. I talked to the investment custodian, and they do not include the date of death in the title for the beneficiary. Do you think this is a problem?

My daughter inherited a Roth IRA from her brother two years ago and Vanguard does not include the date of death in the titling. They do indicate that it is a beneficiary IRA and include his name in the titling. I realize what Ed Slott puts in his books is different, but it does not appear to be an issue.
