Help with Form 8606

This is the first year my husband and I have contributed funds to a non-traditional IRA which we converted to a Roth IRA. We’ve tried to calculate Form 8606 by hand as well as by software and we keep getting different answers. We believe my 8606 form is correct. Here’s the data:

Contributed $5,000, converted to Roth at value of $4,974.71
Had existing SEP-IRA, which we also converted to Roth at value of $4,071.48 (basis $0 as employer funded)

8606 form:
Line 1: 5,000
Line 2: 0
Line 3: 5,000 (yes)
Line 4: 0
Line 5: 5,000
Line 6: 0
Line 7: 0
Line 8: 9,046.19
Line 9: 9,046.19
Line 10: .553
Line 11: 5,002.54
Line 12: 0
Line 13: 5,002.54
Line 14: -2.54
Line 15: 0
Line 16: 9,046.19
Line 17: 5,002.54
Line 18: 4,043.65 (which can be split between 2010 & 2011 taxes)

However, we’re struggling with my husband’s. Here’s the data:

Contributed 6,000 to non-traditional IRA, converted to Roth IRA at 6,078.80
Had existing SEP-IRA with value of $38,205.20 at 12/31/2010 (again basis of $0) – did not convert this to IRA

Our calculations on 8606:

Line 1: 6,000
Line 2: 0
Line 3: 6,000
Line 4: 0
Line 5: 6,000
Line 6: 38,205.20
Line 7: 0
Line 8: 6,078.80
Line 9: 44,284
Line 10: .135
Line 11: 820.64
Line 12: 0
Line 13: 820.64
Line 14: 5,179.36
Line 1: 0
Line 15: 6078.80
Line 17: 820.64
Line 18: 5,258.16

Are we on the right track? Would really like to get our taxes done soon. Thanks for any help you can provide!

Re YOUR 8606: All fine except line 14 cannot be negative and should be -0-. You should also round off each line to the nearest dollar.

Husband’s 8606: All fine until line 15 – it should be -0-, but the rest are correct.

The only real gaffe is that line 15 item which would result in higher total taxable income than is correct. This line 15 was for taxable distributions that were NOT converted, but he does not have these.

Yep, I had a typo. Line 15 should have been labeled Line 16. Thanks for your help.
