I found this thread but can’t see the address. I need to file stand alone 5329’s for 2010-2012 and can’t find any addresses on the form instructions. I am thinking the “not enclosing a check” Form 1040 address for those who live in California.

That address also applies if the 5329 is filed as a stand alone form.

Yes, that is correct. You might check the 2012 instructions to make sure that address has not changed.

Same address where a paper return would be filed. The last page of the 1040 Inst shows the address in the first column.
