FAQ lists three possible actions on an IRA inherited from spouse. Isn’t it also possible to refuse the inheritance & have IRA pass to contingent beneficiaries? If true, how soon does it have to be done?

Yes, that is possible. A qualified disclaimer must be filed with the custodian within 9 months from the date of death.
