Disclaiming
I am familiar with the rules pertaining to disclaiming an IRA – can a beneficiary of a 401(k) disclaim ? Assuming they can — is it dependent on the provisions in the plan document?

Yes, a 401k beneficiary can disclaim unless the plan document stated that disclaimers would not be accepted, and I have not heard of any such plans.
