Forms along with 5329 listed on line 25k
Question 1: filing for excess contributions to Roth IRA for multiple years on 5329’s. They are all completed. Is it necessary to file the additional forms listed on line 25 of the form 5329? (note: Roth excess was distributed after 59 1/2 and 6% tax will be paid.)
Question 2: Can i mail all forms with separate checks in one envelope. Certified mail?
Question 3: What is the best address to send this.

The additional forms are only needed if you are filing the 5329 with Form 1040. Your stand alone 5329s can be sent in one envelope with either a single check or separate checks for each year. Note that while the IRS accepts most of these stand alone forms, occasionally they will request a 1040X for each year even when the excise tax does not affect the rest of the return. Given the added work of several 1040X forms, I would first send in the 5329s without the 1040X.
Yes. Your choice on the separate checks. Not required but given the mess at the IRS now the certified mail with return receipt is a good idea.
Send to your normal filing center printed on the back of the 1040 Inst.
